Please send a picture of yourself with your legal ID (driver's license, State ID, passport, etc). Birthdate must be visible and unaltered.

This is for all countries. Here in the US, it is illegal to sell to a minor or for a minor to have any sort of online payment account. Regardless of what country you are in, if you are under 18, a parent or guardian must fill out this form and send required documents for a commission.

Failure to do so will result in cancelled commissions/quote and cause you to be blasklisted (won't be able to commission anything from me ever) and possible beware put out within the community for other makers.

Anyone found out to have sent false information, will have any payments kept and suit cancelled. If necessary, authorities will be involved.
